1) get HIRED first. But before you actually work, you need to get a work permit by doing the following......
2) get the attached application filled out by
a) your new employer
b) your parent or guardian
c) all your personal information including social security number.
3) Bring the form plus a driver's license, passport, OR Birth Certificate copy to Christopher Welch (x-434) or email@example.com
4) Expect a 24 hour waiting period for processing, return and sign for your work permit.
*** ONLY you can pick up your work permit because you must sign it.
ENTERTAINMENT WORK PERMITS
Entertainment Work Permits must be processed through the Registrar's Office; students must have achieved grades no lower than C's in the preceding term/semester. If a student receives a D or lower, they may reapply for a permit at the next grading term/semester. Please allow 24 hours for processing, and contact the Registrar's Office with any questions.
